Free Initial 30 Minute Consultation

Intake Forms & Labs

Results & Recommendations

Start your transformational journey to wellness with a complimentary initial consultation. During this session we'll discuss the FDN and coaching relationship, your goals, as well as your health history and lifestyle. Next, you will be asked to fill out Intake Forms that will be emailed to you from True Health FDN.

Once you have completed your intake forms and we will schedule an appointment on Calendly to review the forms. We will ask more in depth questions to get a more thorough understanding of your health and symptoms. We will discuss how you can get a head start to improving the way you feel through lifestyle changes. Based on the program you selected I will order the lab kits which will be mailed to your doorstep within 7-10 days. Once you receive the lab kits, we will be available for instruction for how you will provide your lab samples and return to each lab. Upon receipt of your lab results we will begin your wellness transformational journey.

While you wait for your lab results you will have access to the Biocanic Health Portal platform where you can access your account, schedule appointments, receive and send messages and review your personal information. Your next appointment will involve a comprehensive review of the lab data and together you will receive your tailored to your specific needs.

Follow-Up Sessions

We will hold bi-monthly follow-up sessions for progress report Zoom calls which will be booked over the next 3 months on Calendly. Our session is designed to track your progress, address any challenges, and provide ongoing guidance throughout your health and wellness journey.

In our final session we will review your progress, assess any remaining symptoms, and discuss strategies for maintaining your newfound health habits. If we decide you need more time we will discuss an add on program. Remember, achieving optimal health is a lifelong journey, and we'll ensure you're equipped to continue on this journey on your own.

Final Session

The functional labs we use are determined by your clinical symptoms and main complaints. We may bundle 2-6 labs tests at a time. Lab pricing is non-negotiable and out of the FDNP's control and price is determined by the laboratory. However, we will always make it as affordable as possible and choose labs according to your needs and finances. Functional Diagnostic Nutrition® Practitioners (FDNP), have access to thousands of functional lab tests. The labs we use are certified labs in the United States and Canada with strict requirements for efficacy by the State Board of Health.

One-on-One 3 Month Commitment Packages

• Before the appointment we analyze your intake forms and at the appointment ask in-depth questions about your symptoms. This is a deep dive into health history, lifestyle and the Metabolic Chaos® Scorecard.

• During the appointment we discuss specifics about your main health complaints, you get to ask questions and we discuss which labs to use and why.

• An individualized protocol is created for you on day one and then adjusted after we receive lab results.

• 15% off medical grade pharmaceutical grade supplements and other health products at our online store.

• Appointments are online using Zoom or a phone call.

• Bi-monthly progress report Zoom calls. Additional calls are an hourly rate of $200 per hour.

• We do not accept health insurance.

• Check with your insurance company to see about making payment with HSA/FSA. It helps to have a letter from your doctor that our services are medically necessary.

It is extremely important that you are ready to make lifestyle changes. We have had much success overcoming health issues for ages 15-88. However, the level of success depends on many factors such as environment and compliance with the program which are the clients responsibility. We have no control on how committed you are, but we are here to help motivate you in achieving your goals.
